Co-pays


Copayments will be collected at the time of check-in. Payments can be made via cash, check, or credit card.


Insurance Claims


Associated Physicians, LLP files insurance claims on behalf of our patients, but the prompt full payment of the account remains the responsibility of the patient.

 

Though we may accept payment directly from an insurance company, any amount billed but not paid by insurance is the patient's and/or guarantor’s responsibility. Health insurance contracts are agreements between the insured (subscriber/patient) and the insurance company. Please pay the remaining balance and contact your insurance company if you believe there is an error with the claim.


Understanding your Benefits


We will work with you to determine if your coverage is accepted at our clinic. We are not privy to all of the benefits related to your particular plan. Health insurance contracts are agreements between the insured (subscriber/patient) and the insurance company. Please check with your insurance if you are unsure if a particular service will be covered; we cannot quote benefits. We recommend doing this prior to your appointment.

 

Insurance Referrals


Some insurance plans require the patient to obtain a referral or prior authorization from his or her primary care physician prior to seeing one of our physicians.  It is your responsibility to understand the provisions of your policy and to obtain a referral or prior authorization if one is necessary.  If you are unsure about the provisions of your policy with respect to referrals, you should contact your insurance company's customer service department.

 

Self-Pay Patients

 

We offer a 25% self-pay discount to patients who do not have any health insurance coverage and pay for their services out-of-pocket. Patients that have health insurance coverage but choose to use our services out-of-network or request that we not bill their health insurance are responsible for the entire amount of their bill. 

Special Circumstances


Normally, payment of your bill is due within 15 days of the patient balance appearing on a statement. However, our billing representatives will work with you to arrange a payment plan if special circumstances prevent you from making a full, timely payment. Failure to pay may result in a disruption to your care.​

No Show Policy

We appreciate you choosing Associated Physicians, LLP. We schedule our appointments, so each patient receives the right amount of time to be seen by our providers and staff. That is why it is very important that you keep your scheduled appointment with us and arrive 15 minutes early to complete the check-in process.
 

As a courtesy, we attempt to provide each patient with an appointment reminder (via text, phone, or email) to confirm their appointment one week prior to the appointment. A second appointment reminder is sent the night before the appointment. To facilitate this communication, we ask that you update us if any of your contact information is changed.
 

If your schedule changes, and you cannot keep your appointment, please contact us at (608) 233-9746 to reschedule your appointment as soon as possible; this allows us to accommodate patients who are waiting for an appointment. As a courtesy to our office, as well as the other patients who are waiting to schedule with our physicians, please call at least 24 hours before your scheduled appointment to cancel.

If you do not come to your scheduled appointment or are greater than 7 minutes late, we will assess a $25 “no-show/late arrival” service charge to your account. This “no-show/late arrival” service charge is not reimbursable by your insurance company. We also cannot guarantee that you will still be able to be seen as we must respect patients who are scheduled after your appointment.
 

If you do not come to your new patient/establish care visit, you may reschedule one more time with AP; however, you will not receive priority over other new patients that are already scheduled.
 

We understand that unforeseen emergencies occasionally happen. If one of these events occurs prior to your appointment, please call the clinic at (608) 233-9746 to inform our staff as soon as you are aware of the impact on your appointment. The fee may be waived for a single event.
 

After three consecutive no-shows to your appointment, our practice may terminate its relationship with you.

While most patients will never see this fee, it is important that everyone understands the importance of attending all your appointments, and communicating with us in a timely manner if your schedule changes.
