This list MAY NOT include all of our in or out-of-network providers. Please check with your insurance provider to confirm network status and coverage of our services. Our staff DOES NOT have access to the details of your benefit plan and coverage.

Do you have an EPO plan?

EPO stands for “Exclusive Provider Organization” plan.  As a member of an EPO, you can use the doctors and hospitals within the EPO network but cannot go outside the network for care.  There are no out-of-network benefits meaning if the patient goes to a non-EPO provider, his or her services will be out of pocket.

 

WPS and Alliance both have EPO plans that Associated Physicians does not participate in so please make sure that you check your in-network providers prior to selecting your physician.

Medicare Patients


We are happy to have the opportunity to provide care for Medicare patients. As you know, Medicare is a government-administered program designed to ensure that people in need have access to health care. To monitor this program, the government has several processes and forms in place that may be new to you. We are required to strictly adhere to the Medicare requirements detailed on this site.

Medicare "What's Covered" app

 

Not sure if Medicare will cover your medical test or service? Medicare’s free “What’s covered” app delivers accurate cost and coverage information right on your smartphone. Now you can quickly see whether Medicare covers your service in the doctor’s office, the hospital, or anywhere else you use your phone.

 

The app delivers general cost, coverage and eligibility details for items and services covered by Medicare Part A and Part B.  Search or browse to learn what’s covered and not covered; how and when to get covered benefits; and basic cost information.
