Welcome to Associated Physicians!

Thank you for the opportunity to be your partner in healthcare. At Associated Physicians we strive to care for our patients in a friendly and efficient manner.


Please schedule your next appointment before leaving the clinic. This will help us serve you in a timely and efficient manner. If you need to reschedule or cancel an appointment, please contact us at least 24 hours in advance so that we may use your appointment time to accommodate other patients. If you are late for an appointment, we may ask you to reschedule.



If you have a change of insurance, address, or other information, please notify us as soon as possible so that your claims are processed correctly. If you wish to update your demographic or insurance information at the time of an appointment, please arrive 15 minutes early in order to re-register.  Click here to download the Patient Registration Form.


After Hours

In the event of a medical emergency, please dial 9-1-1. If you have an urgent question or concern, please call the main clinic telephone number and our answering service will take your call. They will then contact our physician on-call and s/he will call you back.​



Before you run out of prescription medication, please call your pharmacy and request a refill. They will contact our office for you. Or, you may notify us that you need a refill at your visit.

Referral Program

We Love Our Patients!

At Associated Physicians we love our patients and we want to express our appreciation. We know that you tell your friends and family about the great care you get here and we want to thank you for that.

  • Referrals: If you are a current patient and would like to refer a friend or family member, we'd like to send you a token of gratitude! Please have that person let us know that the referral is from you by asking them to request and fill out a referral card at their first appointment.

  • Reviews: Another way to share your experience is through online reviews. Please take a moment to write an online review on Google or Yelp
