This list may not include all of our in or out-of-network insurance providers. Please check with your health insurance provider to confirm network status and coverage of our services. Our staff does not have access to the details of your health benefit plan and coverage.

*This plan is discount only and does not make the insurance carrier part of the contracted network. Plan holders will receive in-network rates, but services will be applied to out-of-network benefits.

*Quartz Community Plan is an out-of-network plan for our clinic. The new Quartz Cigna PPO Plan will not cover care at Associated Physicians.

Czy masz plan EPO?
EPO oznacza plan „Exclusive Provider Organization”. Jako członek EPO możesz korzystać z usług lekarzy i szpitali należących do sieci EPO, ale nie możesz wychodzić poza sieć w celu uzyskania opieki. Nie ma żadnych korzyści poza siecią, co oznacza, że jeśli pacjent uda się do dostawcy spoza EPO, jego usługi będą dostępne z własnej kieszeni.
