My Chart & Insurance
As a Community Connect Partner of UnityPoint Health-Meriter, Associated Physicians uses UnityPoint's EPIC electronic health record system for our patient information. This means that you also have access to your Associated Physicians patient information through My UnityPoint.
We accept a number of insurance providers and recommend downloading the Unity Point My Chart application to your desktop and/or mobile device so you can view test results, make appointments, and speak with your physician directly.
Flu & Covid Vaccines Now Available
We are currently able to give flu vaccines to children and adults during their regular office visits. For respiratory season we will be giving FLUZONE IIV3 (trivalent), FLUBLOK RIV3 (recombinant trivalent), and Moderna Covid vaccines for ages 6 months -11yrs and 12+ years.

The American Red Cross is experiencing an emergency blood shortage. Over the last 20 years, the number of people donating blood through the Red Cross has fallen by about 40%. When fewer people donate blood, disruptions to the nation’s supply are harder to overcome.
