The best way to obtain your medical records, request medical records, or discuss results with your nurse or physician is to login to your MyChart. Everything is kept in the electronic records system and is easily accessible from your computer or mobile device.

PDF FORMS
General
If you do not have acces to your MyChart, we do have fillable PDF versions online. These forms should be filled out and sent directly to HIM@apmadison.com. Please feel free to also call us with any questions, or contact us here: https://www.apmadison.com/contact.
You may send these PDF forms electronically to our Patient Access Department, or in-person at our clinic. The following are estimated turn-around times for various requests:

5-7 business days to complete forms

3-5 business days for CDs/Images

14 business days for Release of Records

48 business hours for Rx refills (non-controlled)

3 business days for Controlled Substances
